Forms on sale are;
- General Nursing (3 years) – Agbor, Warri and Eku
- Basic Midwifery (3 years) – Asaba and Sapele
Admission Requirements
Five (5) Credits at SSCE at not more than two (2) sittings, including;

Forms are obtainable at various Colleges of choice, Agbor, Warri and Eku and the Department of Midwifery Asaba and Sapele at a non-refundable fee of ₦15,000 (Fifteen Thousand Naira only).
